Week 4 90 minute grocery store tour with Registered Dietitian
Week 5 30 minute nutrition consultation with Registered Dietitian
30 minute training session with Personal Trainer
Week 6 30 minute phone consultation with Registered Dietitian

Membership to the Center for Health & Fitness is included in your WellBeing program.

Assessments
Nutrition Consultation: You’ll meet with CHF’s registered dietitian and receive a complete nutrition assessment, including a detailed analysis of your medical history, diet and lifestyle habits. Together you’ll develop an action plan tailored to meet your individual wellness needs.

Fitness Assessment: One of our highly qualified personal trainers will discuss your exercise history and perform a fitness assessment.

Follow-Up Nutrition Consultations
You will meet again with CHF’s registered dietitian to perform a follow-up nutrition assessment, track your progress and make modifications to your plan where necessary. The program also includes a healthy grocery store tour and cooking demonstration.

Personal Training
You will have six (6) personal training sessions where your personal trainer will develop a customized exercise plan to help you achieve your goals.
